Enoxaparin Sodium
Enoxaparin sodium is a low-molecular-weight heparin (LMWH) used to prevent and treat blood clots.
Common uses
Prevention of DVT/PE, especially after surgery or during hospitalization
Treatment of DVT and pulmonary embolism
Acute coronary syndrome such as NSTEMI/unstable angina
Prevention of clotting during some dialysis procedures
Route
Usually given by subcutaneous (SC) injection, commonly in the abdomen.
Do not give intramuscularly (IM).
Important nursing points
Check the prescribed dose carefully—dosing differs for prophylaxis vs treatment.
Assess for bleeding, bruising, hematuria, melena, or bleeding gums.
Monitor platelet count for possible heparin-induced thrombocytopenia (HIT).
Check renal function (especially creatinine/eGFR) because dose adjustment may be required in significant renal impairment.
Avoid rubbing/massaging the injection site after administration.
Air bubble in the prefilled syringe: Some enoxaparin prefilled syringes are supplied with a small air bubble intentionally. For those products, the manufacturer may instruct you not to expel the air bubble before SC injection, because removing it can cause loss of medication. Follow the specific product's instructions.
