5. Original & Copy of valid visa s and arrival stamps to OECD member countries for the past 5 years (USA and other visas like Canada, Japan, Australia, New Zealand and Korea).
6. Employment Certificate Original (must include applicants position, date hired, compensation, address, office landline & email address). (mobile number not allowed).
7. Bank Certificate Original (must include account type, current balance, account opening date, 6mos average balance).
8. Original ITR / BIR form 1701. (With personal Tin Number) ADDITIONAL REQUIREMENTS EFFECTIVE : March 01, 2015.
9. Bank Statement (original or certified true copy of bank statements/passbook for the last 3 months).
10. Leave of absence certificate.
11. Marriage contract -NSO (photocopy)
12. Birth certificate -NSO (photocopy)

9. Original ITR / BIR form 1701 ( with personal Tin number ) . ADDITIONAL REQUIREMENTS EFFECTIVE : March 01, 2015.
10. Bank Statement (original or certified true copy of bank statements/passbook for the last 3 months).
