The mistakes I see most often with fire lighting are using wood and logs that are too big and not having prepared enough wood in the first place.

Keep your fire material in three sizes.
1) Dry tinder. this can be wood shavings or any easily combustible material. I keep a small amount on me whenever I am in the wilderness.
2) Kindling/Starter wood, Twigs (broken), finely split wood,
3) Main fuel logs.
