ALL INDONESIA Declaration Form

ALL INDONESIA Declaration Form” start from 18 August until 30 September 2025.
ALL Indonesia Declaration Form will pre-launch trial mode to use starting on 18 August until 30 September 2025 and official launching will be start on 01 October 2025.
Three - 3 International airports will come as the pilot project such as CGK, SUB and DPS.
During this transition period, previous QR code still available to use such as
> QR Immigration,
> Custom/Quarantine (E-CD) and
> Satu Sehat application
> These declaration form will be terminated on 01 October 2025 and it will be replaced by “All Indonesia Declaration Form”.
Passengers should complete fill in “ALL Indonesia Declaration Form” before arriving at all International airport in Indonesia. It takes approximately 3 minute to answer all the question given – total 33 fields. It is recommended to fill the form within 3 days prior to departure to avoid the queues and congestion that will occur at the Arrivals Area.
Maximum 10 persons (Family/Group) can submit in “ALL Indonesia Declaration Form”.
If the passengers who arrive at International airport in Indonesia, have not completed submission “ALL Indonesia declaration form”, they will be rejected to do clearance at immigration, even though they are holding APEC, E-VISA and paid VOA on arrival.
