1. one visa application form for Work Permit (IMM 1295) along with family information form (IMM5645) to be filled online
Use of Representative form(lMM 5476), if applicable
2. Statuary Declaration of common-law Union(lMM 5409), if applicable
3. Two photographs, 35 × 45mm in size with white background and face should cover 80% of photo area.

5. Valid Labour Market opinion (LMO) Certificated acceptation du Quebec (CAQ) for cases destined to Quebec (Optional)
6. offer of employment or contract for job in Canada stating duration of employment, nature of work to be performed and salary to be paid.
■ 7. Work Permit to be arranged and sent by the Canadian Counterpart Company
8. Covering Letter in detail from the company in Canada stating the employment proof, purpose of the visit of the applicant, duration of the stay. (Letter to be addressed to the VISA officer, Canadian High Commission, Nigeria)
9. Evidence that meets the requirement of the Job offered:-
10. Proof of Personal Funds or sponsors bank statement with an affidavit of support
11.Copies of appointment letter
