"BORACAY TOURIST REQUIREMENTS "
From GCQ/MGCQ Areas Only

1. Access https://touristboracay.com/ click Choose "BORACAY" in the Where to page.
2. Fill out the required fields and submit. Online Health Declaration. (Atlest 24 hours before the departure flight).
☝Please do not Leave any fields Blank.
☝You may input N/A if item is not applicable. A confirmation with instruction will be displayed after submission.
3. Save a copy of the HEALTH DECLATION CARD (HDC) on your smartphone or have it printed.
4. E-mail the following documents to

OHDC-Family Name, First Name. (ex: OHDC- Dela Cruz, Juan)
☝Registration for multiple tourists and their documents can be sent in one email. (Just include a listing name of the tourist).

5. Wait for the email response ( Successful or unsuccessful) of the final Validator.
Processing take less than a day after receipt of complete documents.
6. If processing is successful a copy of HDC with Tourist QR code will be sent back. Present this HDC with Tourist QR Code to authorities in the borders for scanning. ( Keep the Tourist QR Code with you all the times. Have a printed or digital copy. The QR Code will be used in accessing services in Boracay Island.
7.If processing is unsuccessful, resubmission of requirements or clarification may be required.

BAGUIO REQUIREMENTS

VISITA REGISTRATION:
create an account on ( https://www.visita.baguio.gov.ph/ ), with your valid ID.
on your account, schedule a visit at least a day before your travel.
only 2,000 individual registrations will be allowed for now, following an ETA reservation system.
travel requests are approved after verification of booking by accommodation establishment, contact your AE for updates.
travel only once (QR-coded Tourist Pass) QTP is issued; registration upon arrival is not allowed, strictly no QTP no entry!
FULL VACCINATION:
for adults, upload and bring your vaccination card/certificate, full vaccination will mean 2 weeks after 2nd/last dose.
unvaccinated or partially vaccinated adults are not allowed.
unvaccinated or partially vaccinated minors aged 12 to 17 must present negative results of Antigen or RT-PCR test conducted within 72 hours prior to arrival or during health screening at the City Triage.
minors aged 11 and below shall be tested at the option of their accompanying parents or adult guardian.
TRIAGE UPON ARRIVAL:
after the checkpoint, tourists must proceed to the triage for health screening and verification of QTP and documents.
symptomatic visitors, regardless of vaccination status or test result, will be immediately tested and isolated, and referred to health officials for further action and advice prior to visitor’s return to point of origin.
OTHER IMPORTANT REMINDERS:
Always follow minimum health & safety protocols, wear mask and observe distancing at all times.
Observe existing city laws: no littering, no smoking, vehicle number coding within CBD, and others.
