LACKING, MISLEADING OR NO WRITTEN INFORMATION TO THE
UNDERLINED PORTION OF THE SUBMITTED FORM IS SUBJECT
FOR TERMINATION AND NO DOCUMENTS WILL BE RETURNED
TO THE APPLICANT/S.
NOTE : PLEASE PUT N/A IF THE UNDERLINED PORTION IS NOT APPLICABLE. PLEASE MAKE SURE THAT THE SIGNATURE ON THE APPLICATION FORM AND THE PASSPORT ARE IDENTICAL.
