Ang tatay mo ba ay hindi nakasulat sa birth certificate mo? Blank, unknown or N/A ang nakalagay? Kung ikaw ay isinilang sa taong 2005 hanggang sa taong kasalukuyan narito ang mga hakbang na gagawin para maisaayos ito.

Kung ang mga magulang ng isang bata ay hindi kasal, ngunit kinikilala ng ama na ang bata ay kanya, maaaring gamitin ng bata ang apelyido ng kanyang ama (at ang pangalan ng kanyang ina bilang kanyang gitnang pangalan) sa ilalim ng Republic Act 9255.

Anong gagawin:

Magdala ng PSA copy ng birth certificate sa Civil Registry office.
Magdala ng notarized supporting documents: Affidavit of Admission of Paternity,, Affidavit to Use the Surname of the father, at iba pa.
Ang local civil registry office ay magsusumite ng petisyon sa PSA para sa pag-apruba.
Kakailanganin mong mag-publish ng notice sa hindi bababa sa dalawang publikasyon sa loob ng dalawang linggo.
Kapag naaprubahan, magdaragdag ng anotasyon sa sertipiko ng kapanganakan upang ipakita ang mga pagbabago (upang isama ang bagong apelyido ng bata).
