❤️Rewarding to watch - Last week I was onsite helping a company conduct their benefits open enrollment. We were onsite onboarding employees with a new Memphis DPC 🥼HealthPlan. It was so rewarding to see the employee's disbelief when we told them they would have ✅NO deductible, ✅NO copays for 🩺doctor or 💊Rx, and ✅NO ded for surgery AND their costs were going ⬇️DOWN.

A drug rebate is the return of part of the purchase price by the pharmaceutical manufacturer to help offset the drug’s high retail price to the payer.

Prescription drug are paid by the manufacturer to the pharmacy benefit manager (PBM), which then shares a portion with the health insurer or the self-funded employer. But sometimes the self-funded employer sees none or very little of the rebate, despite being the payer.

Why are you, the payer, not getting all of the rebates since you – not the – are paying for the drugs?

An Rx could save you hundreds of thousands if not millions on your company's Rx spending each year.

21 percent of medical diagnoses reviewed by physicians at the Mayo Clinic are 100 percent wrong and only 12 percent of diagnoses are fully accurate, according to a published study from Mayo.

Yet no insurance companies require a second opinion from an independent third-party physician. With one in five diagnoses completely wrong, why is nothing done to protect your employees and your plan from inappropriate care and unnecessary costs?

Imagine buying a steak dinner and a glass of wine from a menu with no prices; three weeks later you get a bill for $1,045. Who would tolerate such a thing? Yet insurance companies allow doctors and hospitals to do exactly that with health care services.

It’s impossible to learn the price before a procedure or treatment. Why do the insurance companies not demand price transparency from hospitals and physicians so that employees can compare prices in order to be smart consumers – and save your plan money?

Not all doctors and not all hospitals are created equal.

But how do you and your employees know which surgeon produces the highest quality outcomes and which surgeon is facing three malpractice lawsuits? Or which hospital has a nearly zero infection rate and which one infects seven percent of patients with MERSA? The insurance companies have ready access to provider quality scores and metrics; why do they
allow your employees to make what could be a life-or-death decision without the right information?

How can employees be smart consumers with their health care when they don’t know provider quality?

What other product or service, other than health care, costs more every year while you and your employees get – at best – the same plan or, too often, higher out-of-pocket expenses and lesser coverage? Yet every year your broker brings you a renewal increase.

Would you accept this from your office manager, purchasing agent, or supply chain manager on any other purchase?

Your plan should be paying only for appropriate and allowed medical charges. A proactive Payment Integrity strategy can identify disallowed and fraudulent claims over the past 24 months. Most important, the process can recover all or most of your annual overspend – usually 7-10 percent of your annual health care spend – for those two years and put that capital back on your balance sheet.

Does your benefits advisor/broker provide you with strategies designed to reduce the frequency & severity of claims within your company? You might question their efficacy unless you’ve been getting annual reductions in your health insurance rates.

In a recent conversation with Mark Holland, he best describes the need for creating clear strategies to help combat the rising cost of healthcare.

It is a Cost+ thing, but what I would like to get to with our prospects and clients is not selling them RBP or Cost+. These are just terms. Instead, I would sell our professional consulting services. We can then help by building a strategy for the client or prospect that works.

With what is going on in our broken marketplace, the client/prospect needs the opportunity brought to them to help control or cut their costs. Clients and prospects could see their healthcare costs cut by as much as 50%. By helping our clients create a long-term strategy and helping them manage that strategy on an ongoing basis, we can help maximize the savings for the plan and its members. That could look like PPO, RBP, Cost+, or a hybrid of all of those. The strategy used is a matter of what is best for the plan. Advisors do not repeatedly ask for a client's claim data to show them how best to use that data to reduce costs. The first thing I suggest is hiring a consultant. An advisor can do a deep dive into your plans and help you build a better strategy toward controlling costs.

If your current advisor is not having this kind of conversation with you, you are just receiving quotes and nothing more. Even if you are using five different brokers, they are quoting the same thing. This continued course is why the healthcare system stays broke.

If what you have is not getting fixed, it is time employers adopt a new strategy.

It is not just about cost!

Open enrollment is right around the corner, and as you start to decide on insurance benefits for your employees, many of you struggle to communicate these benefits in an engaging manner.

Effective communication about benefits is good for business; employees are invested in their benefit plans more than ever before. Aflac found that more than 50% of American workers reported higher anxiety about healthcare costs beyond what their insurance covers.

With the recent pandemic, rising mental health crises, and rising healthcare costs, employees view their benefits package as a foundation for choosing a job. Therefore, accurately sharing employee benefits with employees is fundamental to recruitment and retention. Eden Health found that 49% of employees will begin searching for a new job within the year if dissatisfied with the benefit plans offered.

Despite the known correlation between retention and benefits education, many employer communications are not hitting the mark and leave their employees stuck in analysis paralysis. A survey from LegalShield found that almost 50% of employees felt unprepared to make informed decisions during open enrollment.

The soaring cost of health care is harming the ability of many employers to recruit and retain workers. In a survey of over 150 employers, 8 in 10 say healthcare costs affect their ability to remain competitive, while three-fourths say these expenses crowd out wage increases. The survey also found that affordability by the recruit was a contributing factor. Drug prices (93%), high-cost claims (87%), and hospital costs (79%) are the most significant cost drivers of employer-sponsored health benefits coverage for recruits, employees, and their families (1). Decision makers let's do something different starting today.
